How can I create a Influence Vaccine Consent Form to send to my patients when they book a flu vaccine appointment? 

Solution

You can use the HotDoc Influenza vaccine consent form to capture patient consent when a patient books a flu vaccine appointment at your practice.

We recommend setting this up in the Vaccinations > Flu section of your Dashboard. Please ensure you link your relevant flu vaccine appointment types to your consent form, in order for this send to patients when they book that appointment type. 

If you have not completed the vaccines set up in the Dashboard, please complete this for your flu vaccine appointments, and you can enable the consent form during this setup. Please see Getting Started with Flu Vaccinations.
