HotDoc Forms, in conjunction with the HotDoc Appointment Calendar, allows you to send a new patient registration form ahead of a patient's appointment.

Send Form using HotDoc Online Bookings
HotDoc will automatically send a New Patient Registration Form in the confirmation email of a New Patient booking:
- If a new patient makes a booking online, their appointment request will be sent to your Sidebar. Select Create New Patient to accept the booking:
- HotDoc will send the new patient a confirmation email with a Get Started button for them to fill in the form:
Send using the HotDoc Appointment Calendar
The HotDoc Appointment Calendar allows you to opt a patient into HotDoc communication consent and send a From when you are adding an appointment:
- When adding a new appointment, select '+ New Patient':
- Enter the patient's details and tick both checkboxes next to Patient consents to receive SMS and email and Send New Patient Form:
- Click Save, and HotDoc will send a new patient form request via email, or via SMS if the patient does not have an email address:
How to check the status of a Form request
The patient's appointment in the HotDoc Calendar will show an icon if a Form request has been requested and if the patient has submitted the Form:
- Form Pending
When HotDoc has sent the patient a request to fill in the form via SMS or email, a blue form icon will display. Hover over the appointment to see the 'Form pending' status:
- Form Processing
When the patient has submitted their Form, a yellow form icon will display. Hover over the appointment to see the 'Form processing' status:
The patient's Form submission will now appear in your HotDoc Sidebar for processing: - Form Completed
When the Sidebar submission has been actioned, a green form icon will display. Hover over the appointment to see the 'Form completed' status:
