The following article explains our recommendations for how you can create and share the MyMedicare Registration form to patients with HotDoc.
When patients open the HotDoc MyMedicare form, it will clearly indicate 'Optional.' Once patients submit the form this will appear in the HotDoc Sidebar and saved as a PDF in the patient's file in your Practice Management Software.
Your practice will then be required to complete the MyMedicare registration process for the patient, by uploading the information collected in the form into the MyMedicare system.

Our recommendations for capturing patient registrations
The following is our recommendations for how you can use HotDoc to capture patient registration for MyMedicare:
- Create the MyMedicare Registration form using the template in your Dashboard

Use our MyMedicare Broadcast SMS template to automatically add your form and send this to your eligible patients.
- Alternatively you could also add the link for patients to register directly through their Medicare Online Account

Easily convert the form into a QR code for patients to scan and complete while waiting for their appointment.
- Copy the URL link of the form so that you can send this in any other way you require
- Enable the HotDoc Inform Campaigns to promote your practice and encourage patients to register through their Medicare online
- View the MyMedicare resources available on the Department of Health website.

What happens once a patient submits a form
Once a patient submits the HotDoc MyMedicare Registration form, this will appear in your HotDoc Sidebar. Click Menu > Forms to view all submitted forms:
If we have a link to the patient file in your practice software, the PDF will save in the patient file automatically.Otherwise you will need to click Add to [practice software] and this will appear. Alternatively, you can download the PDF manually and save this to your desktop.

For Best Practice: The PDF will be saved to "Correspondence In" section of the patient file
- For Medical Director: The PDF will be saved to "Documents" tab in the patient file
- For Zedmed: The PDF will be saved in the Results Inbox in Clinical

From here, you will be required to complete the MyMedicare registration process for the patient, by uploading the information collected in the form into the MyMedicare system. We recommend copying and pasting the information directly from the PDF.
