How to create and share the MyMedicare Registration form in HotDoc

The MyMedicare Registration form helps your practice collect patient consent for MyMedicare, making it easier to manage and track registrations.

Setting up MyMedicare Registration Forms can help your practice:

Once set up, eligible patients will automatically receive your MyMedicare Registration form when they book linked appointment types, allowing your practice to easily collect patient consent and complete their registration in the MyMedicare system.

The patient journey

HotDoc sends the MyMedicare Registration form to eligible patients via push notification, email, or SMS, depending on available contact details. The form is voluntary, requires patient consent, and includes guidance to register only if your practice is their regular clinic, along with links to official MyMedicare resources.

Patients receive the form based on your form settings, linked appointment types, and eligibility criteria: they must be existing patients with a valid Medicare card, not already registered in your clinical software, and not have received the form in the past 120 days.

Additionally, patients may be prompted to register through the MyMedicare Inform Promotion, which directs them to the official MyMedicare website rather than your clinic’s form.

Once a patient submits a form, it can be found in the Medicare Hub under MyMedicare Forms.

How else can I share the MyMedicare Registration form with my patients?

In addition to automatically sending the form to eligible patients for the appointment types you have selected, you may wish to share the form in other ways. HotDoc provides a range of methods to share the form. For example, you can:

  • Use our MyMedicare Broadcast SMS template to send the form proactively to additional eligible patients using the built-in MyMedicare Status Filter. Alternatively, you could add a link for patients to register directly through their Medicare Online Account.

  • Easily convert the form into a QR code for patients to scan and complete while waiting for their appointment.

  • Copy the URL of the form so that you can send it manually by email or SMS.

  • Enable the HotDoc Inform Campaign to promote your practice and encourage patients to register through their Medicare online.
