I want patients to fill in the consent form when they book a COVID-19 Dose 2 and Dose 3 appointment. How can I do this?

Solution

Currently, if the patient completes the consent form before their Dose 1 appointment, we will not send a consent form for Dose 2 & 3 appointments, as consent forms are not mandatory and verbal consent can be obtained. 

If you would like to add your consent form to your appointment types, you can add this to your Dose 2 & 3 appointments by adding the URL in the pre-appointment information. To do this: 

1. In your Dashboard go to Forms > Forms and click Preview next to your COVID-19 consent form:
   
   
   
   
2. Highlight the URL link in the search bar and copy this:
   
   
   
3. Paste the URL in your Dose 2 & 3 pre-appointment information section in your appointment types. See How to edit pre/post appointment information for how to do this.
