How do I include my practice details on the letterhead of the PDF Form Submission?
Solution
When a patient submits their form, HotDoc automatically downloads a PDF version of the form into your practice software and also gives you the option to manually download a PDF.
In some instances, such as your "Transfer of Medical Records" requests, you might want this PDF to include your practice details so that the other practice knows where to send the records or how to follow up with your practice. You can do this by enabling the 'Include letterhead in submission PDF' setting.
How to Include letterhead in submission PDF

Log in to your HotDoc Dashboard and on the left-hand side click on Forms > Forms.
- Click Edit next to your form, then select Settings

Under Form Submission Settings tick 'Include letterhead in submission PDF'

You're now good, and next time you download this type of form submission PDF, the header will include the following practice details:
- Your practice name
- Address
- Phone number
